Employment Waiver Form

This document serves as a waiver of liability between Eternal Life Harvest Center hereinafter referred to as the "Company," and the individual named in the "Employee Name" field in the form provided below, hereinafter referred to as the "Employee," in relation to the Employee's upcoming trip to Knoxville, Tennessee for work purposes.

The Company acknowledges that the Employee will be traveling to the USA for work, and the Employee acknowledges the inherent risks associated with international travel, including but not limited to, exposure to unfamiliar environments, cultural differences, and potential health and safety hazards.

In consideration of the Company permitting the Employee to undertake the aforementioned travel, the Employee agrees to release, indemnify, and hold harmless the Company, its officers, directors, employees, agents, and representatives from any and all claims, liabilities, demands, actions, or causes of action of any nature, whether in law or equity, arising out of or in connection with the Employee's travel to the USA. The Employee understands that this waiver and release does not affect the Employer's obligation to provide compensation and benefits to the Employee as stipulated in the employment contract or required by law.

The Employee expressly understands and agrees that this waiver includes, but is not limited to, any claims for personal injury, illness, property damage, loss of income, or any other loss or damage suffered by the Employee during the course of travel.

The Employee further acknowledges that this waiver extends to any claims that may arise out of the Company's negligence, whether active or passive, and that the Employee is assuming full responsibility for any risks associated with the travel.

This waiver shall be binding upon the Employee, their heirs, executors, administrators, and assigns. It is the Employee's responsibility to obtain adequate travel insurance coverage to mitigate potential risks during the travel.

This Agreement shall be governed by, construed, and enforced in accordance with the laws of Tennessee, United States of America, without regard to its conflict of laws principles.

By signing below, the Employee acknowledges that they have read and understood the terms of this waiver, and voluntarily and willingly waive any claims against the Company as described herein.
